Rights:
To be respected as an individual and to receive competent and
compassionate care.
To know the names of all health care providers and staff members
caring for you and to have complete
information regarding diagnosis, prognosis and treatment in language
you understand.
To be able to access all information necessary to make an informed
consent prior to procedures and
treatments. You have the responsibility to notify your health
care provider if you do not understand.
To refuse consent for treatment and to receive full information
regarding the consequences of this action.
To have your medical information kept confidential by your insurance
plan and health care provider.
Confidentiality laws and professional rules of behavior allow
your health care provider to release medical
information only when it is required for your care, required by
law, necessary for the administration of your
plan or to support your health care programs or operations that
evaluate quality and service. We may also
summarize information in reports that do not identify you or any
other participants specifically.
To have your health care provider give you information about your
medical condition and treatment options
regardless of benefit coverage or cost. You have the right to
receive this information in terms you understand.
To review your or your child’s medical records and your
bill and to have them explained.
To respectful response to reports of pain.
Patient Responsibilities:
To provide all information about your or your child's health as
completely and as accurately as possible.
To respect the privacy of other patients and families.
To meet the financial obligations for your or your child's care.
To discuss with your health care provider any questions, concerns
or dissatisfaction you have about your or
your child's care.
To obtain the information you need about your health care plan,
including information about covered services
and non-covered service and any costs you will be responsible
to pay.
Keep scheduled appointments and notify the office ahead of time
if you are going to be late or miss an
appointment.
To notify your employer and health care provider's office as soon
as possible about any changes in family
